Terms of Participation & Liability Release
Organization: Tr3nch 5v5 Football League
1. AGREEMENT TO PARTICIPATE
By signing this agreement or checking the acceptance box on the Tr3nch 5v5 Football League (“The League”) website, I affirm that I am the participant or the legal parent/guardian of the minor participant named in the registration. I acknowledge that I am entering into a binding contract with The League, its affiliates, sponsors, officers, volunteers, and agents (collectively, the “Released Parties”).
2. MEDIA CONSENT AND LIKENESS RELEASE
I understand that The League events may be photographed, filmed, or otherwise recorded. I grant The League and its partners an irrevocable, worldwide, royalty-free right to use my (or my child’s) name, image, voice, and likeness in any media formats, now known or hereafter created (including social media, website, television, and print).
- Usage: These materials may be used for marketing, promotion, commercial advertising, and news coverage without compensation to me.
- Ownership: All footage and imagery remain the sole property of The League. I waive any right to inspect or approve the finished product.
3. MEDICAL AUTHORIZATION & FITNESS
I attest that the participant is in good physical condition and has no medical limitations that would make participation in 5v5 football unsafe.
- Emergency Care: In the event of an injury or medical emergency, I authorize The League staff to secure medical treatment. I agree that I am solely responsible for all costs associated with such medical care and transportation.
- No Duty to insure: I understand that The League does not provide health or accident insurance for participants.
4. SAFETY & EQUIPMENT PROTOCOLS
I acknowledge that I have read The League’s safety standards.
- Headgear Policy: I agree to wear (or ensure my child wears) the mandatory protective headgear approved by The League (soft shell helmets/headgear) during all gameplay.
- Inspection: I agree to inspect the field and equipment prior to play. If I observe any unsafe conditions, I will immediately remove myself (or my child) from play and notify a League official. Failure to do so constitutes an acceptance of the field conditions.
5. ASSUMPTION OF RISK
I fully understand that 5v5 football is a physical sport that carries inherent risks. These risks include, but are not limited to: collisions with other players, contact with the ground/fixtures, weather conditions, equipment failure, and negligence of others.
- Severity of Risk: I acknowledge these risks can result in minor injuries (scrapes, bruises), serious injuries (concussions, broken bones, ligament tears), or catastrophic injuries (permanent disability, paralysis, or death).
- Voluntary Participation: I voluntarily assume all such risks, known and unknown, and accept full responsibility for participation.
6. WAIVER OF LIABILITY AND INDEMNIFICATION
To the fullest extent permitted by law, I hereby release, discharge, and covenant not to sue Tr3nch 5v5 Football League or the Released Parties for any claims, demands, losses, or damages related to my participation.
- Hold Harmless: I agree to indemnify and hold the Released Parties harmless from any liability, legal fees, or costs arising from my participation or my conduct during the event.
- Scope: This release applies to injuries or damages caused by negligence of the Released Parties, field conditions, or fellow participants.
7. JURISDICTION
This agreement shall be governed by the laws of the State of Florida. Any legal disputes arising from this agreement must be settled in the courts of Miami-Dade County [or insert your specific county]. I expressly waive any right to a trial by jury.
